As Mother&rsquo\;s Day approaches our Louisville dentist team at Boulder Valley Dental would like to recognize all the amazing mothers and women in our lives!
\n\nOur annual $5000 Mothers Day Smile Makeover time is here. We are excited to be able to provide this service would love to keep the tradition alive for many years to come. \;Please nominate someone in your life that would be deserving of a little pampering of their own for a change.

\n\nPlease read the rules carefully\, as only those who are nominated correctly can be considered.
\n\nThe rules are as follows:
\n\n- \n
- Nominations can include a mother\, wife\, sister\, cousin\, aunt\, neighbor\, coworker\, school teacher&hellip\;basically anyone that has made a difference in your life or you care about. \n
- Nominations are made by \;completely filling out the \;nomination submission form on our website homepage\, \;or by \;emailing our team info@bouldervalleydental.com \;a photo of your nominee doing what she does best\, along with a brief summary of why this woman in your life would be deserving of a smile makeover. \n
- All nominees must be 18 years or older. \n
- Nominate as many wonderful women as you like\, just not yourself. \n
- Each submission must include the name\, age\, and a brief description of why this individual is deserving of the makeover. Please also include photo of her doing what she does best. \n
- Only Colorado residents are eligible. \n
- Relatives of our team members are not eligible. \n
- All entries are due by Sunday\, May 1st 2016 11:59PM. -Four (4) finalists will be selected from the nominees by the team at Boulder Valley Dental based on the criteria of dental need and deserving nature\; and will be announced on our FB page May 2nd 2016. \n
- The four finalists will then be put back to you for final selection via likes/votes that will take place over the week of May 2nd 2016 on our FB page. \n
- The nominee with the most likes/votes at 11:59PM on Saturday May 7th 2016 will be the winner\, and announced on our FB page Mother&rsquo\;s Day (May 8th 2016) \n
- Dental services received by the winner may total up to $5000. Treatment options are solely at the discretion of the providing doctor and can only be services offered at Boulder Valley Dental Center. The treatment plan must be accepted by signing a &ldquo\;Consent to Treat&rdquo\; form prior to any work being done. \n
