DESCRIPTION:The Healthy YOU program is a FREE health improvement program designed to help individual manage chronic conditions or simply improve their overall health. It includes a 7-week evidence based\, self-management program that was created by the Stanford University medical School. The program has been shown to improve a participant's exercise level\, symptom management and to general self-reported improvements in general health\, fatigue and social activities. Patients are supported by highly trained and experienced Health Coaches for 2 1/2 months utilizing motivational interviewing skills to build on the problem-solving and confidence building skills taught in class.
